It has been prepared by Shirtliff \u0026amp; Co Wholesale Manufacturing \u0026amp; Retail Chemists of 66 Goldhawk Road, London.  There is a motif logo on the left of the envelope.  It has a telephone number 1018 Hammersmith.\u003cbr\u003e\u003c\/p\u003e\n\u003cp\u003eInside is the original prescription which is dated 22 June 1908.  It has a list of items which were bought including cotton wool, plasters and Horlicks malted milk and the prices are on the right.  It also says \"paid with thanks 22\/6\/08 Shirtliff \u0026amp;
